Athletic Physical Information
Any student who wished to participate in Athletics will need a cleared Sports Physical on file with the school. They must also have these on file to try out for a sport.
These must be submitted EACH YEAR for the student athlete to remain eligible for play.
Any doctor’s office will have a Sports Physical form, but for our IHSAA Physical Packet (which contains other forms with required signatures) please click the link below:

All students will need a new physical for 22-23. Please take the time to fill out the needed information. Even if you are not sure if your child will try out for a sport, it is highly recommended they still get a physical before the year starts to avoid the rush of getting one last minute and possibly missing tryouts for any sport.
Like last year, we are using Eventlink for all forms. We will be paperless. Please follow the directions below to create an account, register your child, and access all forms. You will be able to fill out the forms online and upload the doctor’s physical.
To Create an Account and start filling out forms:
- Go to www.eventlink.com and click create account
- Search for and add + (add) your school
- Complete the form with parents information
- Be sure to click add contact
- Complete registration
Register Athlete and Accessing Forms:
- Click Eventlink Registration Tab
- Add your student + (add) and save. If athlete is already registered, just select the name and continue below.
- Click Eventlink Registrations and click on the 2021-2022 Athlete Registration button.
- Complete all of the REQUIRED forms. You must select at least one season to choose a sport.
Subscribe to School Calendars
- Click on “My Calendars”
- Select “Manage My Calendars” from the drop-down
- Add + (add) the calendars you want to follow and SAVE
